I believe the supposed "wealth and healthcare advantage" of CA and Hawaii may be overstated. Clean air locales worldwide seem to have a health advantage (note Blue Zone locations Okinawa, Sardinia, Loma Linda). https://www.epa.gov/clean-air-act-overview/progress-cleaning-air-and-improving-peoples-health Parts of the clean air hypothesis are obvious, others perhaps more subtle. HI and CA were known to have less incidence per capita of Covid-19, but is this all due to clean air?
